Septocaudal deviation

These are “columellar” septal deviations, that is, low deviations of the nasal base, known as deviations of the vestibular area, which cause difficulty breathing through the nose and, in some cases, aesthetic changes.

In these cases the septal deviation of the nasal base was corrected. The changes achieved, although millimetric, greatly improve nasal breathing and appearance.

A photograph of the nasal base is very useful for assessing the condition.

At the consultation, the nasal base, columella and caudal end of the septum are examined and correlated with the breathing difficulty the patient reports. Sometimes the deviation is subtle at first glance and shows more clearly on the basal view.

Correction aims to recentre the caudal septum and improve airflow, while protecting tip stability and nostril symmetry. Depending on the case, it may involve sutures, limited resection or support grafts.

When it coexists with other aesthetic or functional changes, septocaudal deviation is addressed in the same operation, so shape and breathing are treated together.
